Reverse Incorrect Ratio
What it means
With reverse selected, the transmission computer found that the ratio between input and output speed does not match reverse. It usually shows up as delayed reverse engagement or slipping when backing up a slope, from a slipping clutch or band that applies reverse, or low pressure.

Why does the car hesitate before backing up?

Can the shifter adjustment be the cause?
Yes in some cases, especially after service. If the selector does not fully reach reverse, full pressure may not be applied, so check the adjustment before internal repairs.
