What it means

The engine computer has seen the upstream oxygen sensor voltage on bank 1 stay above the allowed limit. The cause may be a short feeding extra voltage into the signal wire, a failed sensor, or a very rich mixture that makes the sensor see almost no oxygen.

How to diagnose5

Check the fuel trims; strongly negative trims mean the mixture really is rich, so find the source of extra fuel.
Unplug the sensor and watch the reading; if the voltage stays high with no sensor, the fault is in the wiring or computer.
Inspect the connector for moisture or corrosion bridging its terminals.
Check the injectors and measure fuel pressure against the manufacturer specification.
If the wiring and mixture are fine, the sensor is the most likely cause.

Clean the connector or repair and separate the shorted wires.
Replace the upstream oxygen sensor if its signal is proven high internally.
